    • Job Requirements
    • NBSTA required unless candidate can provide CST employment since 2012
    • Primary Source Verification of professional license and/or certification required within 30 days of start date
    • Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or proof of immunity required
    • Must have documentation of TB skin test screening within 3 months of start date
    • Physician Statement signed by MD/DO/NP/PA required within 365 days of start date
    • Upload attestation indicating date completed and findings for background checks
      • Additional Details
      • AHA, Red Cross, CHSF or Military certifications preferred for BLS
      • N95 fit testing must be completed within 365 days of start date and updated annually
      • COVID-19 vaccination required with no exemptions considered
      • Flu vaccine required during flu season (Sept 15th - April 30th)
